ICCB Budget Modification Submission Form

This form is for submitting Budget Modification Forms for GATA-Exempt/non-Amplifund-based grants. If you have any questions, please contact the ICCB support staff for your grant and CC iccb.grantpayments@illinois.gov.

The deadline for budget modification requests is 60 days before the end of the award term. Budget modification reviews may take up to 30 days for the Grantor to complete.

If your grant is in AmpliFund, you need to submit your budget modification (amendment) within AmpliFund. If you need assistance, please contact your ICCB program support.

Please do not submit other requests, such as Extension Requests, through the Budget Modification Submission Form. This form is only for budget modification requests. For other types of request, please contact your program support staff.

Some grants may require the submission of more than one document as part of your budget modification request. For example: the Taking Back the Trades grant requires the submission of both the Budget Modification form and an updated Budget Template.
