Permission to Withhold Parent/Guardian Directory Information Form

Release to OSPI for Higher Education - for High School Families Only
Release of high school students' parent or legal guardian directory information to the state Office of the Superintendent of Public Instruction for the purpose of sharing with higher education institutions in Washington state is guided by RCW 28A.150.515 and RCW 28B.10.041.

Parent/guardian directory information includes:
- Parent/guardian name
- Address (mailing)
- Phone number (primary phone)
- Email address
If parents/guardians do not want their directory information released, please complete the form below on or before October 15 of the current school year.

Each parent/guardian in the household should complete this form if families do not want their households' information released.

This process is separate and independent of the release of student directory information. If you do not want your student's directory information released to higher education institutions or as permitted by the Family Educational Rights and Privacy Act (FERPA), please review the information on the Permission to Withhold Student Directory Information website.
