Pre-License Agreement

Crescent City Driving School, LLC. (herin after referred to as the "school"), agrees to provide qualified, licensed instructions foe the purpose of abtaining a Louisiana Class E Driver's License.

The school does not guarantee the securing of a Lousiana Class E License to operate a motor vehicle. The test will consist of 40 or more questions including signs, lane markings and signals. A score of 80% or higher is required to pass. Each student shall be allowed to take the test twice the first day. If the student fails the test both times, he/she may retest twice the next day thereafter until he/she passes. Upon successful completion of the test the student shall deliver the sealed envelope to the OMV. Any test that is not in a sealed envelope when presented to the OMV shall be rejected and the student will be instructed to return to the driving school to retake the knowledge test. Any student caught cheating will forfeit his test and will not be allowed to retest for thirty days. The OMV has the right to retest you on both the knowledge and skills tests. 

DEPOSITS: There will be a $100 non-refundable fee if for any reason a student decides not to attend class or drops out of the school. $50 fee if a student does not show up on the day of class and has to be moved to another class. $25 fee if a student does not show up for BTW lessons.

Cost: 14 Hour Course $450 (6 classroom hrs, written test, 8 hours BTW) RST-$60

Cost: 38 Hour Course $500(30 classroom hrs, written test, 8 hours BTW) RST-$60

Any grievance(s) not resolved by the school may be forwarded to the Department of Public Safety and Corrections, Office of Motor Vehicles, Attention CDL and Commercial Driving Schools Unit, P.O. Box 64886, Baton Rouge, LA. 70896

ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM

I hereby assume all risk of participating in any/all activities associated with CRESCENT CITY DRIVING SCHOOL, including by way of example and not limitation, any risk that may arise from negligence or carelessness on the part of the persons or entities being released from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.

I Certify that I am physically fit, have sufficiently prepared or trained for participation in this activity, and have not been advised to not participate by qualified medical professional. I certify that there are no health-related reasons or problems which preclude my participation in this activity.

In consideration of my application permitting me to participate in this activity, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

 

A.    I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to, liability arising from negligence or fault of the entities or persons released for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including traveling to and from this activity, THE FOLLOWING ENTITIES OR PERSONS: CRESCENT CITY DRIVING SCHOOL and/or their directors, officers, employees, representatives, and agents, and activity holders, sponsors, and volunteers.

B.    INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned in this paragraph from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

 

I acknowledge that CRESCENT CITY DRIVING SCHOOL and their directors, officers, volunteers, representative, and agents are NOT responsible for the errors, omissions, acts, or failures to act of any party or entity conducting a specific activity on their behalf.   

I acknowledge that this activity may involve a rest of a person’s physical and mental limits and comes with the potential for death, serious injury, and property loss. The risks include, but are not limited to, those caused by terrain, facilities, temperature, weather, condition of participants, equipment, vehicular traffic, lack of hydration, and actions of other people including but not limited to, participants, volunteers, monitors, and/or producers of the activity. The risks are not only inherent to participants, but are also present for volunteers. I hereby consent to receive medical treatment which may be deemed advisable in the event of injury, accident, and/or illness during this activity.{signature}

I understand while participating in this activity, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose by the activity holders, producers, sponsors, organizers, and assigns.

The Accident Waiver and Release of Liability Form shall be construed broadly to provide a release and waiver to the maximum extent permissible under law.

 

I CERTIFY THAT I HAVE READ THIS DOCUMENT AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.
