By submitting this form, I acknowledge that I am voluntarily participating in the CHC World Missions trip to Aruba. I understand that participation in this trip involves certain risks, including, but not limited to, travel, recreational activities, outdoor activities, and ministry-related events.

I voluntarily assume all risks associated with this trip, including the possibility of personal injury, illness, property damage, or loss that may occur during travel, lodging, transportation, or any activities related to the trip.

In consideration of being allowed to participate, I hereby release, discharge, and hold harmless CHC World Missions, the Congregational Holiness Church, Inc., and any of their officers, employees, volunteers, or agents from any and all claims, liabilities, damages, or expenses arising from my participation in this trip, including claims resulting from negligence, except for claims arising from gross negligence or willful misconduct.

I also grant permission for CHC World Missions and its representatives to seek medical treatment on my behalf in the event of an emergency and understand that I am responsible for any costs incurred.
