child(ren), to participate in Midweek and related activities at First Baptist Church of Terrell. I/We also grant permission for my/our child to ride the bus or van of First Baptist Church so my/our child can have transportation from school.

I/we understand and agree that transportation to First Baptist Church of Terrell for my/our child to participate is being provided as a convenience and courtesy to me/us. I/We understand that it is my/ our responsibility to ensure that a responsible person is available to pick up my/our child(ren) after Wednesday Night Activities at First Baptist Church Terrell.

I/we, the undersigned, waive any claim against First Baptist Church of Terrell, its ministers, staff members, or any adult agents acting in a supervisory role regarding supervision of Midweek events and activities or transportation to or from such activities. I/We also acknowledge that the information provided on this form is correct and accurate to the best of my/our knowledge and that I/we will notify First Baptist Church of Terrell of any changes.

I/we, the undersigned, grant permission for any minister, staff person, or authorized Children’s Ministry leader of First Baptist Church of Terrell to take necessary steps to obtain emergency medical or dental care due to an emergency. I/We understand that First Baptist Church will seek to contact me/us as soon as possible should such a situation arise.

First Baptist Church, its ministers, staff members, and authorized adults will be held harmless for reasonable acts taken to transport and care for my child and for obtaining emergency medical care for my child, if necessary.
