ACCIDENT WAIVER AND RELEASE OF LIABILITY FORM
I hereby assume all risks of participating in any and all programs, sessions, training, camps, or activities offered by K&M Hockey Development Inc. and the K&M Hockey Development High Performance Centre, including but not limited to risks that may arise from negligence or carelessness on the part of the persons or entities being released, from dangerous or defective equipment or property owned, maintained, or controlled by them, or because of their possible liability without fault.
I certify that I am physically fit and have not been advised by a qualified medical professional to refrain from participation. I certify that there are no health-related reasons or problems that would preclude my involvement in any K&M Hockey Development Inc. or High Performance Centre activities. I acknowledge that this Accident Waiver and Release of Liability Form will be used by the organizers and operators of these programs and facilities and will govern my actions and responsibilities during participation.
In consideration of my application and being permitted to participate in K&M Hockey Development Inc. and High Performance Centre programs, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors, and assigns as follows:

I WAIVE, RELEASE, AND DISCHARGE from any and all liability, including but not limited to liability arising from the negligence or fault of the entities or persons released, for my death, disability, personal injury, property damage, property theft, or any actions of any kind which may occur to me, including travel to and from these activities.
THE FOLLOWING ENTITIES OR PERSONS ARE RELEASED:
K&M Hockey Development Inc., the K&M Hockey Development High Performance Centre, their owners, coaches, employees, agents, representatives, contractors, and volunteers.

I INDEMNIFY, HOLD HARMLESS, AND PROMISE NOT TO SUE the entities or persons mentioned above from any and all liabilities or claims made as a result of participation in any K&M Hockey Development Inc. or High Performance Centre activity, whether caused by negligence or otherwise.
I acknowledge that participation in hockey training, on-ice sessions, off-ice training, synthetic ice, treadmill activities, gym use, and all related programs carries the potential for serious injury, including death and personal loss. Risks may include, but are not limited to: ice conditions, synthetic ice conditions, facilities, equipment, temperature, weather, physical exertion, actions of other participants, coaches, volunteers, spectators, and dehydration.
I consent and agree that K&M Hockey Development Inc. and the K&M Hockey Development High Performance Centre, and their coaches, agents, representatives, or volunteers may take photographs or digital recordings of me during participation and use these in any and all media for training, promotional, or educational purposes. I further consent that my identity may be revealed therein or by description, text, or commentary. I waive any rights, claims, or interest related to such media, and I understand that no financial or other compensation will be provided.
This Accident Waiver, Release of Liability, and Image Release is intended to be construed broadly to provide a full release and waiver to the maximum extent permitted under applicable law.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT, AND I SIGN IT OF MY OWN FREE WILL.
