Are there any dietary restrictions or preferences we should accommodate? This helps ensure the child's needs are met during snack or meal times, and when we prepare snacks on the farm.

What are your expectations for the nature school program? What are your goals for your child's learning and development in this setting?
What is your comfort level with outdoor play in various weather conditions, including rain, snow, and mud? Are you prepared for your child to come home dirty?
Are you comfortable with your child engaging in "risky play", within appropriate safety guidelines? (Risky play is different from hazardous play and can be beneficial for children's development.)
What is the best way to communicate with you (e.g., phone, email, text)? In-person meetings can be arranged as needed when provided ample notice of one week.

Signatures and Liability Waiver - Please read carefully.

This section is to clearly outline the inherent dangers and risks associated with participating in nature school activities. I understand that my child is volunteering to participate in farm and outdoor activities, and that he/she will be outside in nature around animals. There are inherent risks that come with being on a farm and in nature. All leaders and staff will make every effort to prevent injuries and ensure activities are safe. I hereby release Acorn Acres Nature School (AKA LEAP Outdoors Farm and Forest School and Acorn Acres Farmstead) and its affiliates, including Owners, Directors, Volunteers, and Parents from any potential harm, injury, and/or death while named child is participating in any tours, farm activities, nature activities, including both travel to and from the location, and while on site, both inside any buildings and outside. While on site, my child may experience encounters with plants and wildlife, rough terrain, weather changes, and potential for injury or illness during physical activity. In case of an emergency requiring medical care, as Parent and/or Guardian of the named child, I hereby authorize the diagnosis and treatment by a qualified and licensed medical professional, of the minor child to prevent further endangerment of the minor's life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort. I give consent for my child to be transported to the hospital if deemed necessary for appropriate treatment. This will be done only after a reasonable effort has been made to reach the signed caregiver by phone and/or text. This waiver will apply to each participant enrolled on this form if multiple children are registered together. Release authorized on the dates and/or duration of the applicable school year.

Confirmation: BY ACKNOWLEDGING AND SIGNING BELOW, I UNDERSTAND I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE. THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE. I am signing on behalf of my minor child and relinquishing the same rights as if signing for themselves.
