IMPORTANT NOTICE
1. Submission of the Club Application and Consents Form confirms acceptance of all required commitments for all teams representing the club in TYSL.
2. This form must be submitted and approved by League Management prior to any team being bracketed or scheduled for games.
3. All registration fees must be paid in full before teams will be eligible for bracketing or scheduling.
4. THIS FORM MUSTBE SUBMITTED BY AN AUTHORIZED CLUB REPRESENTATIVE.

CLUB CONSENTS
SCHEDULE COMMITMENT: I confirm that all teams registered by our club are committed to participating in all scheduled Saturday games, as well as any Sunday rain-out makeup dates as determined by the league.*

ROSTER INTEGRITY: I understand that any player written in by hand on the official match card/roster who is not shown in the GotSport system as approved player (on the team roster or a club pass player) will result in an automatic forfeit of the match and a $50 fine per ineligible player.*

LATE FORFEIT PENALTY: I acknowledge that any game forfeited within 48 hours of the scheduled kickoff time will result in a $100 fine, to be paid by the team before their next game. Teams that continue to forfeit games will be subject to further penalties.*

RISK: I affirm that by attending or participating in the league that all coaches, players, club members, and parents affiliated to the club, assume all of the risks and hazards incidental to the league and release, absolve, hold harmless and waive all claims against TYSL and all its' respective officers, agents, and representatives (and, if applicable, the owners and lessors of any facilities used to host the league) arising from traveling to, participating in, or returning from scheduled league activities.*

NO PLAYER RECRUITMENT: I understand that improper player recruitment is prohibited by TYSL, which includes any effort or activity whatsoever (by any coach, club official, or team parent) that could reasonably be construed as attempting to encourage, induce, entice or solicit a player registered with one club or team to transfer to another club or team (by any form of communication or action, whether written or verbal). If a parent is found to have improperly recruited a player from another club or team, his or her conduct will be attributed to the club for whom they were recruiting. Penalties may be imposed for improper player recruitment which may include, but not be limited to, the suspension of coaches (or other club officials), the ejection of a team or club from the league, and/or fines levied against the club involved.*

IMAGE RIGHTS: I understand that by attending or participating in the league the club grants permission to all league officers, agents, and representatives the right to use photographs and/or video images taken of club members, players, and/or their supporters, and/or any other representatives of the club, for any purpose, including the promotion or advertising of the league or any other future events.*
