We require this pre-registration form to be filled out and submitted before November 30, 2025.
I will submit the $500.00 pre-registration fee by January 31, 2026, and players fees of $125.00 per player minus the $500 pre-registration fee by Feb 28, 2026.
I will etransfer these funds to chc55plus@gmail.com or make alternate arrangement with the CHC committee using the contact us form on this site.
