  • MEDICAL RELEASE In case of emergency, when the parents or guardians cannot be reached, I grant permission to St. Mark Catholic Church to provide and/or obtain medical attention for my child(ren). I realize that the recommendations of the emergency medical service will determine to which hospital my child will be taken if it is an extreme emergency. I further authorize that medical personnel of the hospital to care for and/or treat my child. Also, I agree on behalf of myself, to hold harmless and defend St Mark and the Diocese of Baton Rouge from any claim arising from or in connection with any illness or injury. Please check one.*
  • PUBLIC INFORMATION & COMMUNICATIONS I grant permission for St. Mark Church to use my child(ren’s) name(s), as listed in the PSR registration and any and all media, whether now known or hereafter devised, including but not limited to internet postings, newsletters, flyers, posters, brochures, promotional materials, merchandising, advertisements, fundraising letters, annual reports, press kits, submissions to journalists, websites, internet sites, mobile applications, social media, social networking sites, and other print and digital communications.*
  • CIRCLE OF GRACE Educates and empowers children and young people to actively participate in a safe environment for themselves and others, while being taught in the framework of our Catholic faith. The Diocese provides Circle of Grace training to children in grades K-12 in (diocesan, parochial and interparochial) schools and in church parish schools of religion. Circle of Grace lessons bring God and our Catholic beliefs into the center of child protection by teaching: God’s presence and assistance in difficult and confusing situations as an essential component in the Pre-K–gr. 12 curriculum. A philosophy that can be used at all times that reinforces God’s real presence in our lives, respect for self and others, and skills for good decision making. The sacredness of each person and how relationships are called to be experiences of divine love. Circle of Grace contributes to the protection of our young children and affirms them as beloved children of God. Therefore, I grant permission for St. Mark Church to present the Circle of Grace curriculum to my child(ren). If not I will complete an Opt-out form before the date of the lesson.*

  • ST. MARK PSR ATTENDANCE & PROGRAM POLICY

    Students attending any PSR Program are expected to be present for ALL classes. We recognize that there may be circumstances that prevent a child from attending a particular class. However, your child’s religious education should be made a priority.

    SUMMER PSR GRADES 1-9: · If a child has an excused absence during summer PSR, makeup work will be provided. (eg. Illness, death of a family member, etc.)· If a child has one unexcused absence, during summer PSR, makeup work will be provided. · Makeup work will be issued on the last day of classes. Students must achieve a 75% or greater score on all makeup work.· If a child has two or more unexcused absences during summer PSR, the student will be enrolled in the Homeschool Program for that year.· Students enrolled in the summer PSR program are expected to attend grade-specific retreats and class Masses throughout the year. Confirmation Students: · 7th, 8th & 9th grade students will receive grade-specific requirements at the Parent Meeting.· They will also receive grade-specific information for retreats and class Masses at the Parent Meeting.

    HOMESCHOOL PSR 1-8 GRADE ONLY*: · We will use the online homeschooling program My Catholic Faith Delivered for the 1st—6th grade homeschool option. Called to Be Catholic books will be used for the 7th—9th grade homeschool option. 7th, 8th and 9th grades will also attend prep dates in person.· Information such as login will come at the end of June. Score percentage for passing the homeschool option is 75% or higher. · Students enrolled in the Homeschool PSR program are expected to attend grade-specific retreats and class Masses throughout the year. I am a registered parishioner of St. Mark. I have read, understand, and will abide by the abovementioned policy.
