I authorize ArtiSonata Studio staff to pick up my child from school on all scheduled After-School Program days during the 2025–2026 school year.

I understand that:

- I will provide this signed form to my child’s school.
- ArtiSonata Studio staff may present a copy of this form at pick-up as identification.
- If my child’s schedule or pick-up arrangements change, I must notify ArtiSonata Studio in writing.

This authorization remains valid for the entire school year unless revoked in writing.
I have read and agree to the authorization above.
4. Parent Acknowledgement
I will deliver a signed copy of this form to my child’s homeroom teacher on the first day of school.
