WAIVER & RELEASE
In consideration of being allowed to participate in Slip 'N Slide Kickball at Union Chapel Church, I, the undersigned, acknowledge, appreciate, and agree that:
1. Physical Risk Acknowledgement: Participation in this activity involves inherent risks including, but not limited to, slipping, falling, collisions, water-related injuries, sprains, bruises, broken bones, or more serious injuries.
2. Assumption of Risk: I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the organizers, and I take full responsibility for my participation.
3. Release of Liability: I, for myself and on behalf of my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless Union Chapel Church, its leaders, employees, volunteers, and agents from any and all liability for injury, disability, death, or loss or damage to person or property arising out of or in connection with my participation in this event.
4. Medical Treatment: I give permission for church staff or adult volunteers to seek emergency medical treatment for me (or my child) if necessary. I understand that I am responsible for any medical expenses incurred.
5. Photo/Video Release: I grant permission for any photographs or video taken during the event to be used by Union Chapel Church for promotional or social media purposes without compensation.
6. Minors Only: If participant is under 18, I confirm that I am the parent/legal guardian and I give permission for my child to participate and agree to all terms of this waiver.
