  •       I,      ,   Understand that Next Level Dance Company is not responsible or liable for any injuries that may occur during my child’s enrollment in class. I am aware of the risks of my child participating in physical activity such as dance and tumbling. If such injuries happen to occur, NLDC is not expected to financially support any medical expenses. I also understand that I and my child,    , are to conduct ourselves in a polite and professional manner when representing NLDC, whether it be in the studio or out in public. Any behavior we exhibit that is inappropriate or offensive can result in my child’s enrollment being terminated. I am prepared for the commitment that comes with enrolling my child into dance/tumbling. If my child is absent from classes due to any personal reasons, I am aware that I must still pay my monthly tuition regardless of the number of classes missed in a single month in order to continue my child’s enrollment in NLDC. I also understand that Mrs. Holly may change the date and time of my dancers class practices if a class does not make or if a class needs to be divided due to the number of enrolled students.
