Note from Twilight Camp

Carver County Parks requires an additional waiver for youth to participate in Log Rolling at Twilight Camp. There's some overlap with the contact and health info you've already given Twilight Camp. That info is below, please make any updates that need to be shared with Carver County Parks.

We’ve listed the parent as the emergency contact for Carver County Parks records, but we’ll still use your original parent/caregiver contact info from Twilight Camp registration.

There’s also a consent box below for first responder treatment. Camp staff will continue to follow Twilight Camp procedures for first aid, caregiver notification, and related needs.

I, the undersigned, give permission the aforementioned participant to partake in the marked Carver County outdoor recreation programs on July 21 - 24, 2025

I understand that participants in this program will be involved in various activities both on and off the water during the duration of this program. I hereby release Carver County, their officers, directors, agents, employees and volunteers from any claims, demands, injuries, damages, actions or causes of actions whatsoever arising out of or connected with my child/family participation in the programmed activities. I hereby grant Carver County Parks and to its employees, agents and assigns the right to photograph my dependent/family and use the photo and or other digital reproduction of him/her or other reproduction of his/her physical likeness for publication processes, whether electronic, print, digital or electronic publishing via the Internet.
