If the incident involves a code of conduct issue (e.g. crowd member) please return to the PFL website and use the other NOI form that requires a bond. Notice of Investigations can be lodged for a player, official, or club who is alleged to have committed or engaged in conduct that constitutes a reportable offense where a player has been injured or vilified. No payment is required with this submission. Submission must be received by Monday 9am following the match in line with bylaws and tribunal timelines for that week. You will receive a confirmation email if this form is submitted correctly. While this form enables vilification submission, under the AFL Peek rule any vilification issue can be submitted within 28 days of the alleged offence and this can be done through emailing PFL Competition Integrity Officer Tony Walton on tony@perthfootball.com.au
