If I have authorized my information to be released for the purpose of legal discovery, reproduction and distribution in the course of litigation, I understand that the information used or disclosed pursuant to this authorization may be disclosed and reproduced by the recipient though the course of the pending litigation and may no longer be protected by the Federal Privacy or other such applicable laws.

I, the undersigned, have read and authorize the release of such information as herein contained. I have the right to revoike this authorization in writing at any time except to the extent that action has been taken in reliance upon it. I understand that when this information is used to disclose pursuant to this authorization, it may be subject to re-disclosure by the recipient and may no longer be protected. I hereby release and hold harmless the City of Fort Dodge Human Resources staff from all liability and damages resulting from the lawful release of my personal information. This authorization is voluntary. A copy of this authorization shall be as valid as the original.
