  • In order to receive support you must complete the required viewing and read through the contract below and agree to its terms. You wil then be redirected to Doulado, where you will complete the inquiry process. By submitting this request for support, you are also attesting that you have:

    Access to a reliable/working cell phone for communication with your doula
    Access to reliable transportation to and from your birthing space
    Stable housing or a safe environment for prenatals to be conducted

  • By submitting this form, I/we acknowledge that I/we have read and understand this contract describing the doula services provided through mOBiNurses Inc. and agree to its terms. I/we consent to the release of this and all other information pertaining to me to the person(s) designated as backup for my birth.

    • Doulas do NOT:

      • Perform clinical tasks, such as blood pressure, fetal heart checks, vaginal exams, or other medical procedure that fall under the legal definition of practicing medicine.
      • Provide information, dosages or advice concerning prescription or alternative medication, or medical advice pertaining to you or your baby.
      • Make decisions for you. Your doula(s) will help you get the information necessary to make an informed decision. Your doula(s) will also inform you if there is a deviation from your birth plan.
      • Speak to the staff on your behalf. Your doula(s) will discuss your concerns with you and suggest options, but you or your partner or a family member or friend will speak on your behalf to the clinical staff.
         
        As a client of mOBiNurses Inc. you are entitled to:

      -Up to 8 hours of birth education and planning with your doula

      -Labor & birth support 

      -Immediate postpartum support up to 2 hours

      -Up to 8 hours of check-ins post-birth to ensure you're healing well, managing your new role and have all resources neccessary

      -Virtual access to mOBiStaff for 12 months following delivery

      *Doulas working with mOBiNurses Inc. are considered independant contractors. Any services provided beyond what is stated above is dependent on the terms set by your doula(s) and will not be considered services attached to this mOBiNurses Inc. contract.

       

      Obligation of the doula:

      Your doula(s) will draw on their knowledge and experience to provide emotional support, physical comfort and information to help you make informed decisions during pregnancy, labor and delivery. Your doula(s) can provide reassurance and perspective to you and your partner, make suggestions for labor progress, and help with relaxation, massage, positioning and other techniques for comfort. Your doula(s) work for you, not your caregiver or hospital.

      Backup: Your doula(s) will inform you of any times they may be unavailable for support and will arrange one or more backup doulas to fill in if you so wish. Any potential back-up doulas can be met virtually or in-person to acquaint yourselves.

      Replacement Doula Request: As with any care provider, you are entitled to request a replacement of your current doula(s) for any reason. We will do everything we can to ensure you are matched with a support team that alligns with your ideals and expectations. Unfortunately due to the nature of this work, we cannot gaurantee that a replacement doula will be available upon request. 

       


      Prenatal appointment(s): 

      Your doula(s) prefer to meet with you and your partner or additional support person at least once before labor to become acquainted, explore and discuss your priorities, fears/concerns, and to plan how to best work together. Your doula(s) will also want to know your own best ways of coping with pain and fatigue and how you and your partner foresee working together and with your doula team. These meetings can be done virtually or in-person.

       

      Labor & Birth: 

      Your doula team is committed to supporting you during your labor and birth. 

      Your doula should be called as soon as you believe labor has begun or as soon as your bag of waters has ruptured, even if you do not yet need them. Your doula(s) will typically be with you for in-person support within two (2) hours from the time you ask for their presence. Your doula(s) can answer questions and make suggestions by phone. Except for extraordinary circumstances, your doula or their backup will remain with you throughout labor and birth.

      C-Sections: Your doula(s) will provide support throughout the C-section process based on the protocol of the facility.

       

      Immediate Postpartum Support:

      Your doula(s) will typically remain with you for up to 2 hour after the birth, until you are comfortable and your family is ready for quiet time together. During that time, your doula team will ensure that you are recovering well and settled in. Your doula(s) are available via phone to answer questions about the birth of your baby. 

       

      Postpartum Check-ins:

      Your doula(s) will provide up to 8 hours of support within 6 weeks following the delivery of your baby. These may be in-person or virtual dependant on your agreement.

       

      Client Obligations:

      • Maintain prompt communication during labor to ensure your doula is present in your time of need
      • Treat your support team with dignity and respect
      • Address any concerns or ask any questions as soon as you have them
      • Maintain a sanitary and safe environment for the benefit of yourself and your support team
      • Disclose any communicable diseases that you or a family member may have so that your doula can take appropriate .
         

      Cancelation of services:

      BOTH mOBiNurses Inc. and clients reserve the right to cancel contracted support at any time for any reason by submitting a "Support Resolution" report located in the client app.

       
      Waiver of Liability: As the express condition of the performance of services under this agreement, you agree, on your own behalf and on behalf of all persons or entities you may bind under Ohio law, to waive and hereby do waive any rights, claims or causes of action that you may have against your doula team now and in the future with respect to your pregnancy, birth, postpartum period or the services your doula(s) may provide and/or fail to provide to you. You expressly understand that doulas are not medical professionals and therefore cannot and will not provide you with medical care or advice.


      Dispute Resolution: We agree, in the unlikely event that a dispute should occur between us, that all disputes shall be resolved pursuant to Ohio law and that, except as otherwise indicated below, the parties to this agreement are not entitled to and shall not receive attorneys’ fees and/or expenses arising from any such dispute.

      Responding to Subpoenas and Other Requests for Information: If your doula is required to respond to a subpoena of their records relating to services your doula performed for you, or to testify by deposition or otherwise concerning such services, you agree to reimburse the doula for her time and all expenses incurred in responding to any such demand, including, but not limited to, loss of earnings, appearing at depositions or hearings, or retaining an attorney.
