- Yo doy permiso voluntariamente para que mi hijo/a participe en eventos, clases o actividades que ofrece la Iglesia Piedra Viva/Guitarra en el Barrio (“IPV/Guitarra”) de forma gratuita o por una cuota/donación nominal. Entiendo que debo proporcionar mi propia guitarra para las clases de música.
- Expresamente, junto con mis herederos o representantes legales, renuncio, libero y pacto de no demandar a IPV/Guitarra y cualesquiera agentes o empleados, contratistas independientes, funcionarios, directores, empleados , sustitutos, agentes y sucesores, de cualquier obligación, responsabilidad, reclamación, demanda, costos, y gastos, incluyendo honorarios de abogados, que surjan de, o en conexión con, cualquier daño o lesión, no obstante causados, que ocurren en el curso de o durante o relacionado con la participación en la música o de otros programas/eventos/actividades de IPV/Guitarra.
- Que soy responsable del seguro médico y de accidentes y de cualquier costo médico incurrido debido a una lesión.
- Doy permiso para el transporte médico de emergencia y el tratamiento de su (s) estudiante (s) a su cargo si surge la necesidad (esto puede no ser relevante para la tutoría virtual).
- Soy responsable como padre durante la sesión de instrucción/tutoría virtual de monitorear las interacciones del instructor/tutor con su (s) hijo (s) y ayudar a garantizar que no haya ruidos de fondo o interrupciones durante las sesiones de instrucción/tutoría.
- Yo prometo a animar a que mi hijo practique cada dia la guitarra.
- Concedo a IPV/Guitarra el derecho a tomar fotografías de mí y mi familia en relación con las actividades y eventos que IPV/Guitarra lleva a cabo. Autorizo a IPV/Guitarra, sus causahabientes y cesionarios el derecho de autor, utilizar y publicar la misma en forma impresa y / o electrónica. Estoy de acuerdo que IPV/Guitarra podrá utilizar dichas fotografías de mí o mi familia con o sin mi/nuestros nombre/s y para cualquier propósito legal, incluyendo, por ejemplo, con fines tales como la publicidad, la ilustración, la publicidad y el contenido web.
- Doy permiso que se comparta la información sobre mi hijo al instructor/tutor.
He leído el comunicado de arriba y exoneración de responsabilidad y entiendo completamente su contenido.
- Entiendo que no hay devolucion de la cuota de la inscripcion una vez comiencen las clases.
- Entiendo que el alumno puede perder su cupo si falta a mas de tres sesiones de clase en el semestre y no habra devolucion de la cuota inscripcion.
- Acepto voluntariamente los términos y condiciones establecidas anteriormente. Este acuerdo se mantiene vigente durante el tiempo que participo/participamos en las clases o actividades de IPV/Guitarra.

By checking 'I Accept' below and submitting this form, you, the parent / guardian of the above named student (s), agree to:
- I voluntarily give permission for my child to participate in events, classes or activities offered by Iglesia Viva Piedra/Guitarra en el Barrio (“IPV/Guitarra”) for free or for a nominal fee / donation. I understand that I must provide my own guitar for music classes.
- Expressly, together with my heirs or legal representatives, I resign, release and agree not to sue Iglesia Piedra Viva UMC / Guitarra en el Barrio (IPV/Guitarra) and any agents or employees, independent contractors, officers, directors, employees, substitutes, agents and successors, of any obligation, liability, claim, lawsuit, costs, and expenses, including attorney's fees, arising out of, or in connection with, any damage or injury, however caused, occurring in the course of or during or related to participation in music or other IPV/Guitarraprograms / events / activities.
- That I am responsible for accident and medical insurance and for any medical costs incurred due to injury.
- I give permission for emergency medical transportation and treatment of student(s) in their care if the need arises (this may not be relevant to virtual tutoring).
- I am responsible as a parent during any virtual instruction/tutoring session to monitor the instructor / tutor's interactions with their child (ren) and help ensure that there are no background noises or interruptions during instructional / tutoring sessions.
- I promise to encourage my child to practice the guitar every day.
- I grant IPV/Guitarra the right to take pictures of me and my family in relation to the activities and events that it carries out. I authorize IPV/Guitarra, its successors and assigns the copyright, to use and publish it in printed and / or electronic form. I agree that IPV/Guitarra may use said photographs of me or my family with or without my / our names / s and for any legal purpose, including, for example, for purposes such as advertising, illustration, publicity and Web content.
- I give permission for information about my child to be shared with the instructor / tutor.
- I understand that once clases start there is no refund of the registration fee.
- I understand that if a student misses more than three classes in a semester the student may lose their slot in the class and there will be no refund of registration fees.
- I have read the above statement and disclaimer and fully understand its content. I voluntarily accept the terms and conditions set forth above. This agreement remains in force during the time that I participate / participate in IPV/Guitarra classes or activities.
