I understand that this is a drug- and alcohol-free community. Any violation may result in immediate removal.
I acknowledge that the facility is under 24-hour surveillance for the safety of all residents.
I understand that no unapproved visitors are allowed in the home.

I understand that I am responsible for paying program fees on time each month.
I acknowledge that failure to pay may result in termination of my placement.
