Paying by check?
Do not complete this online form if paying by check. Email the information requested in fields on the form with subject line "UnConference Sponsorship" to admin.naswmn@socialworkers.org. You will receive a drop link in which to add your files. Mail a check payable to NASW-MN to PO Box 92, Backus, MN 56435. Payment and files must be received by end of day on Thursday, October 9, 2025 to guarantee timely processing of your sponsorship. Sponsors that sign up after this date will have their sponsorship processed on a rolling basis as received.
