1. Introduction
I understand that my child is being assessed using the SASC 2025 diagnostic report format. The assessor will collect personal, educational, and health information in order to produce a diagnostic report in line with current professional guidelines and standards (https://www.sasc.org.uk/news/launch-of-the-2025-sasc-new-diagnostic-report-format/).

2. Consent to Assessment and Use of Personal Information
By signing below, I confirm the following:

• I have parental responsibility for the child named above.

• I give permission for my child to take part in this diagnostic assessment.

• I consent to the assessor collecting and using personal, developmental, educational, and medical information about my child for the purpose of completing the assessment.

• I understand that relevant background details (e.g. family history, health, education) may be included in the report, unless I request otherwise.

3. Consent to Report Sharing

I understand that the report will not be shared with any other individuals or organizations without my explicit consent.  

I understand that the report will be emailed to the parent who commissioned it and completed the initial request form for them to share with me.  This is for ease of administration and does not impact on my rights.

4. Rights and Access
I understand that:

• I may request to review or correct factual content in the Background Information section of the report before it is finalised.

• I have the right to withhold or withdraw consent for the inclusion of certain sensitive information.

• I may request a copy of the completed report.

Please note: if parents are unable to agree / consent then it may not be possible to carry out the investigation. 

It is understood that both parents will liaise with each other to ensure that any factual content in the Background Information section of the report is confirmed before the assessment takes place and the report is finalised.
