Students in grades 6 through 12 who take part in a school-sponsored activity, such as a sport or a performing arts program, use this form to pay the annual student activity fee. The fee has two parts. Every participating student pays a base fee once per school year. Students also pay an activity fee for each sport or activity they join. For example, at the standard rate, a student who plays soccer and basketball pays a $25 base fee plus $25 for each sport, for a total of $75.
Because the base fee is paid only once a year, students who already paid it in the fall do not pay it again. If they join a new sport or activity in the spring, they pay only the activity fee for that sport or activity.
These fees help cover the cost of offering programs that build student engagement, leadership, and school spirit. Once the fees are paid, students receive an activity sticker for their school ID. Sixth graders may join the Middle School Play, but interscholastic sports begin in grade 7.
This form also provides the option to purchase of an All-Sports Dragon Pass, which grants admission to all regular-season home sporting events during the 2026-27 school year. The price of the pass is reduced as each sports season concludes.

Free Lunch Program
No fees are required
Students who qualify for the Free Lunch Program do not need to complete this form. These students may check in at the Middle School office or High School athletic office to receive their activity sticker.
Payment and Admission
Once the base and activity fees are paid, an activity sticker will be issued and placed on the student’s school ID. This sticker grants free student admission to all regular-season home sporting events, as well as performances of the Middle School Play, High School Fall Play, and High School Spring Musical.
