This consent form is to be completed prior to receiving an ingrown toenail checking and care. Our skilled technicians are committed to providing the highest quality of care. However it is essential to understand the nature of the treatment, associated risks, and care instructions.

Aftercare instructions:

Maintain cleanliness by washing the affected toenail with soap and water daily and applying alcohol or peroxide twice daily for sanitization. Avoid tight shoes to prevent pressure on the toenail, and use the provided antibiotic cream to prevent infection. Monitor the area for any signs of increased redness or swelling and seek medical attention if these symptoms occur.
I release Lume Nails LLC (DBA Lume Nails & Spa), its owners, and nail technicians from any legal liability, including financial responsibility for injuries incurred, even if due to negligence, during my treatment.I have been advised to consult a podiatrist or seek medical attention immediately if I experience severe pain, discharge, or excessive redness, especially if I have diabetes.I understand the risks of ingrown toenail cutting, including pain, bleeding, or infection.I understand that the technician will be limited to cutting the nail corner and/or removing dead skin/cuticle to alleviate discomfort.I agree to undergo this service voluntarily and I am informed of the risks.I confirm that I do not have any conditions increasing my injury risk during this service.I acknowledge that while the salon strives to prevent them and accept the minor risks of infection and consent to the treatment
