By signing this form, I acknowledge and agree to the following:

1. I understand that I must be an active member or corporate sponsor of JaxREIA to participate in the JaxREIA Funding Club.

2. I authorize JaxREIA to contact me using the information provided on this form for matters related to the Funding Club.

3. I understand that participation in the Funding Club does not guarantee investment opportunities, borrower placements, or returns.

4. I acknowledge that JaxREIA acts solely as a connector between funders and capital seekers and is not a party to any lending transaction, agreement, or negotiation that may result.

5. I understand that all funding decisions, risk evaluation, due diligence, and loan structuring are my sole responsibility.

6. I acknowledge that JaxREIA does not verify borrower information, underwrite deals, or participate in the negotiation, servicing, or enforcement of funding terms.

7. I understand that participation in an orientation session, course, or class may be mandatory to help me understand how to review deals, protect my capital, and align with the expectations of the Funding Club.

8. I acknowledge that the JaxREIA Funding Club officially launches in September 2025, and no introductions, matchings, or transactions will occur prior to that time.

I have read and understood this disclosure, agree to participate under these terms, and hereby release and hold harmless JaxREIA, its officers, directors, employees, and agents from any liability, claims, or damages arising out of or related to my participation in the Funding Club.
