Participant Waiver and Release of Liability
1. Acknowledgment of Risk
I, the undersigned participant (or legal guardian if under 18), acknowledge that participation in soccer activities involves inherent risks, including but not limited to physical injury, illness, or, in rare cases, death. I voluntarily assume all risks associated with participation.
2. Medical Emergency Authorization
I authorize the organization, its staff, and volunteers to obtain medical care or emergency treatment deemed necessary for the participant.
I agree to be financially responsible for any medical treatment or emergency transportation provided.
I release the organization from liability for any injury or illness incurred during participation, except in cases of gross negligence.
3. Refund Policy
All payments are non-refundable except in cases where the organization cancels a session or program.
No refunds or credits will be issued for missed sessions, participant withdrawal, or expulsion due to violation of organization policies.
4. Package Expiration Policy
The package of 10 training sessions has to be completed within 10 weeks from the start of the 1st training. ( minimum 1 time a week )
The package of 20 training sessions has to be completed within 20 weeks from the start of the 1st training. ( minimum 1 time a week )
The package of 30 training sessions has to be completed within 30 weeks from the start of the 1st training. ( minimum 1 time a week )
Unused sessions after the expiration period will be forfeited and cannot be refunded or credited.
5. Media Release and Use of Likeness
I grant permission for the organization to take photographs and video recordings of the participant during activities.
I authorize the use of these images and recordings for marketing, promotional purposes, and publication on the organization's website and social media platforms.
I waive any right to inspect or approve the finished product or compensation arising from such use.
6. Code of Conduct
I agree to abide by all rules and policies set forth by the organization.
Disruptive or unsafe behavior may result in removal from the program without refund.
7. Cancellation Policy
If you confirm a soccer training session and cancel with less than 24 hours’ notice, you will forfeit that session.
Please be mindful when confirming your soccer training. There are no exceptions to this policy.
