1. Introduction
I understand that I am being assessed using the SASC 2025 diagnostic report format. The assessor will collect personal, educational, and health information to produce a report following these professional standards (https://www.sasc.org.uk/news/launch-of-the-2025-sasc-new-diagnostic-report-format/).

2. Consent to Assessment and Use of Personal Information

By signing below, I agree to the following statements:

• I am aged 13 or older.

• I consent to the assessor collecting and using my personal, developmental, educational, and medical information for the purpose of completing the diagnostic assessment.

• I understand that background details (e.g. family history, health, education) may be included in the report, unless I or my parent/carer request otherwise.

3. Consent to Report Sharing

I understand that the report will not be shared with any other individuals or organizations without my prior agreement (or that of my parent/carer).  

I understand that the report will be emailed to the parent/carer who commissioned it and completed the initial request form for them to share with me. This is for ease of administration and does not impact on my rights.

4. Rights

I acknowledge I have the right to:

• Review and request changes to factual information in the Background Information section (e.g. health, developmental history).

• Withhold consent for certain information to be included.

• Withdraw my consent at any time before the report is finalised.

Please note: if the individual does not provide their consent then it will not be possible to carry out the investigation. 

It is understood that my parent will check the factual content in the Background Information section of the report is confirmed before the assessment takes place and the report is finalised.
