We are very happy to have you joining us. Please fill out the following information to the best of you ability. Please take special care to ensure that names are spelled correctly as this information will be used to populate many documents along the tour.

Stage management will do its best to honor dressing room preferences. We are often limited by the amount of rooms available space, and it is very likely you will have to share a dressing room at some point on the tour. Indicate below which dressing room would make you most comfortable. Please feel free to add any additional information or talk with stage management directly about any concerns or needs.

Stage management will be helping to coordinate a Birthday Club for the tour. What is it? A simple way to celebrate as a company, individuals participating will provide treats for the company (Cookies, Cake, Pie, Guacamole, etc...) to celebrate the Birthday that falls directly after theirs. Stage management will provide plates, napkins, forks, etc... (We do appreciate a hand in setting it up, and cleaning it up.) On the Birthday, all are welcomed (even if not in the Birthday Club) to gather at half hour to sing and celebrate and eat our treats before we get into costume.

Please provide a photo/headshot that can be used for Security lists, Facesheets, Mirror Tags, etc... This doesn't need to be a professional photo - just something that clearly shows your face and looks like you.
