NHET-Sim Online Workshop Registration

This is the RSVP form for the following NHET-Sim workshops:

• Module S9: Debriefing in simulation

Friday, 12 December 2025 - 1:00-4:00pm (Melbourne, Australia Time, AEDT, GMT +11 hours)

Workshop cost: AU$220 (GST included) 

All NHET-Sim workshop registrants receive 4 months access to the e-learning platform.

To complete your RSVP, read the following and action where necessary:

1) After submitting this RSVP, submit your payment via:

Your place at the virtual workshop is not secured, unless payment is made together with the RSVP.

2) As space for the workshop is very limited, should you be unable to attend, please let us know ASAP by email (med-nhet-sim@monash.edu) so we can allocate the space to another participant.

At the event of inability to attend by the participant after payment has been made, the participant will be offered a credit to attend another future workshop of the same value. 

PLEASE NOTE THAT ONCE YOU SUBMIT THIS FORM, YOU WILL RECEIVE A COPY OF YOUR RESPONSES VIA EMAIL. THIS CONFIRMS YOUR RSVP (RSVP WILL ONLY BE SECURED ONCE PAYMENT IS MADE).
YOU WILL NOT RECEIVE ANY OTHER EMAIL CONFIRMATION FROM THE NHET-SIM OFFICE.

THE NHET-SIM OFFICE WILL ORGANISE E-LEARNING ACCESS (NEW ACCOUNT OR RE-ACTIVATION/EXTENSION OF YOUR EXISTING ACCOUNT) ABOUT 1 WEEK PRIOR TO THE WORKSHOP DAY.

THE NHET-SIM OFFICE WILL SEND OUT A REMINDER AND HOUSEKEEPING EMAIL, INCLUDING ZOOM LINK ABOUT 1 WEEK PRIOR TO THE WORKSHOP DAY.
