IMPORTANT NOTES:

1. No SSFA team club or other winter club jerseys

2. Registering your team does NOT necessary guarantee you a spot. You will be notified if unsuccessful within 72 hours of submission.

3. Format for U6s is 4 vs 4 and from U8s will be 5 vs 5 (inc GK)

4. Menai Hawks Summer Football will be subject to fields being in a safe and playable condition and not subject to excessive damage by our use. We will do our very best to play every week of the summer season but wash outs may occur which will result in loss of play due to ground closure by council. Any wash out rounds will not be played and deemed CANCELLED. We note NO refunds in part or full available due to loss of play (or deregistration).

5. You are required to come as team. NO individuals will be placed to a team.

Players (Senior Teams) are required to register as individuals and pay via www.playfootball.com.au by Fri 3rd Oct 25

Minimum number of 6 Players (U6s Competition) and 7 Players (U8s to Seniors) is required to form a team. Maximum = 10 Players

As a representative/agent of the nominated team above, I apply for registration of the team to participate in the 2025/26 Fast 5's summer football competition operated by Menai Hawks FC Inc. If accepted, the team will bound by the rules of the competition and code of conduct as published. All players must have or obtain a FFA number and must be registered through www.playfootball.com.au before start of the competition. I/we accept that players who do not satisify this condition will not be covered by insurance. I/we confirm that the dates of birth shown are true and accurate. I/we accept that registration will not be accepted until fees are paid in full and completed registration is received. Registration Fees are NON REFUNDABLE. Signing the registration form constitutes acceptance of the agreement with the above. I accept the registration rules and conditions.
