Kalisi kids Art Class Registration Form
All classes include a 20-30 minute craft session, followed by a snack (including a juice box) and play time.

Aprons are provided, but please dress your child in clothes that can get messy.

Parents or guardians must remain on-site during the entire class and are responsible for supervising their children during snack and play time.

While participation in the craft is optional, parents or guardians may relax in the main café or courtyard until the craft session ends and children transition to snack and play areas.

Topic - paint night. Comes with a mini easel, canvas, paint brush and pallet to bring home. Don't worry we have aprons!

KALISI KIDS CRAFT NIGHT PARTICIPATION AGREEMENT, LIABILITY WAIVER, AND RELEASENOTICE: PLEASE READ THIS ENTIRE DOCUMENT CAREFULLY BEFORE SIGNING. BY SIGNING THIS WAIVER, YOU GIVE UP CERTAIN LEGAL RIGHTS.In consideration of my child/ward being permitted to participate in the Craft Night activities at Kalisi Kids, located at [Insert Address], including the use of equipment, toys, and facilities in the kids’ café, main café, and outdoor courtyard, I, the undersigned parent or legal guardian, acknowledge and agree to the following on behalf of myself, my child/ward, and our heirs, assigns, and personal representatives: 1. Activities and Risks: I understand that Craft Night includes a 20-30 minute craft session, snack time (including a juice box), and play time in the kids’ café, main café, and/or outdoor courtyard. Activities may involve the use of scissors, glue, paint, markers, small tools, craft materials, playground equipment, toys, and other items that carry inherent risks. These risks include, but are not limited to: • Minor injuries such as cuts, scrapes, bruises, or skin irritation. • Major injuries such as burns, eye injuries, choking, or allergic reactions. • Property damage, including damage to clothing or personal belongings. • Risks associated with food consumption, including potential allergic reactions or cross-contamination.I acknowledge that these activities may occur in areas not fully controlled by Kalisi Kids and that, despite reasonable precautions, not all risks can be eliminated. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of Kalisi Kids, its employees, agents, or affiliates. 2. Food Allergies and Snack Time: I understand that snacks and juice boxes provided during Craft Night are prepared and/or sourced off-site. Kalisi Kids cannot guarantee an allergen-free environment, and cross-contamination may occur due to factors beyond their control, such as changes in manufacturer formulations. Parents or guardians are responsible for providing accurate and complete information about their child’s food allergies or dietary restrictions prior to participation. Children with severe food allergies must bring their own safe, ready-to-eat snacks (refrigeration and heating are not available) and may not participate in food-related activities if deemed unsafe. I assume all risks related to food consumption and release Kalisi Kids from liability for allergic reactions or food-related incidents. 3. Parental Responsibility: Parents or guardians must remain on-site during the entire Craft Night event and are responsible for supervising their child during snack and play time. While parents or guardians may relax in the main café or courtyard during the craft session, they must be available to monitor their child afterward. I understand that Kalisi Kids staff are not responsible for supervising children*
