I understand that, if I am employed by the schools within the Diocese of Bridgeport, I will be required to submit to a state and national criminal history records check for a period of thirty (30) days from the date of my employment. I further understand that I will be required to submit to fingerprinting and that my fingerprints will be forwarded to the Federal Bureau of Investigation (FBI) for a national criminal history records check, all at my expense.
I further understand and agree that, if I have been convicted of a crime which has not been disclosed to the Schools within the Diocese of Bridgeport, the School may immediately terminate my contract of employment (certified employee position), or dismiss me (non-certified employee position), in accordance with the provisions of Public Act No. 93-328.
I hereby certify that I have included in this application all current and former employers and their contact information who are boards of education, charter school governing councils, interdistrict magnet school operators or other employment that involved contact with children. I further consent to the disclosure of information from the State Department of Education regarding the applicants eligibility for a position requiring a certificate, authorization or permit; information about the Departments knowledge about findings of substantiation against the applicant for child abuse, neglect or sexual misconduct; and information received by the Department that the applicant has been convicted of a crime or has pending criminal charges.
I hereby authorize any and all law enforcement agencies, current and former employers, credit agencies, and academic institutions to supply any information regarding my background to the Glastonbury Public School System and to its agents and employees, and I hereby release all such former employers, law enforcement agencies, credit agencies and academic institutions, their agents and employees from any liability arising from the supplying of such information. If you are 15-17 years old and this is your first job, you must provide proof of your eligibility to work.
