📄 RFC Participation, Photo & Liability Release Agreement
PLEASE READ CAREFULLY. THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR RIGHTS. BY SIGNING OR CHECKING BELOW, YOU AGREE TO THE FOLLOWING TERMS:
1. Voluntary Participation & Assumption of Risk
I am voluntarily participating in activities hosted by Richmond Floorball Club (RFC), including the Atlantic Floorball Open. I acknowledge that participation involves vigorous physical exertion and potential exposure to elevated indoor temperatures (85 °F / 29 °C or higher), which may lead to heat-related illness, injury, permanent disability, or death. I fully assume these risks, including those from RFC’s negligence.
2. Waiver, Release & Covenant Not to Sue
In consideration of being allowed to participate:
I release RFC, its volunteers, officers, sponsors, partners, and venues ("Released Parties") from any liability for injury, damage, illness, or death arising from RFC Programs — even if caused by negligence.
This release applies to all claims before, during, or after participation.
3. Indemnification & Hold Harmless
I agree to indemnify and hold harmless the Released Parties against any claims or expenses resulting from my participation or breach of this agreement.
4. Medical Fitness & Emergency Treatment
I confirm I am medically fit to participate and will not play under the influence. I authorize RFC to secure emergency care on my behalf and accept responsibility for any associated costs.
5. Photo, Video & Media Release
I grant RFC and its partners the irrevocable right to capture and use my likeness (photo/video/audio) for any lawful RFC-related purpose in any medium, now or future. I waive the right to review such material and release RFC from any related claims. If I wish to revoke future consent, I will notify RFC in writing in advance.
6. Age Certification
I certify that I am at least 14 years old as of August 31, 2025, or that I am a parent/guardian signing on behalf of a minor participant with legal authority to do so.
7. Governing Law
This agreement is governed by the laws of the Commonwealth of Virginia. If any provision is found unenforceable, the rest remains valid.
8. Entire Agreement & Electronic Consent
This release represents the full agreement. Electronic agreement (e.g., checkbox or typed name) is legally binding.
