Informed Consent and Acknowledgement
I hereby give my approval for my child’s participation in any and all activities prepared by RLM during the "Secret Place". I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless RLM Ministries and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.
In case of injury to said child, I hereby waive all claims against RLM Ministries . including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.
Consentimiento Informado y Reconocimiento
Por la presente, autorizo la participación de mi hijo/a en todas las actividades organizadas por RLM durante el "Lugar Secreto". Asumo todos los riesgos y peligros inherentes a la realización de las actividades y libero, absuelvo y eximo de toda responsabilidad a Ministerios RLM y a todos sus respectivos funcionarios, agentes y representantes por lesiones que sufra dicho/a hijo/a durante el viaje, la participación o el regreso de las sesiones del campamento.
En caso de lesión del/de la hijo/a, renuncio a cualquier reclamación contra Ministerios RLM, incluyendo a todos los entrenadores y afiliados, todos los participantes, agencias patrocinadoras, anunciantes y, si corresponde, a los propietarios y arrendadores de las instalaciones utilizadas para realizar el evento.
