Faith Formation Family Registration Form
  • Dear Parent or Guardian,

    We are excited for you and your entire family to join us for the 2025-2026 faith formation programs at St. Joan of Arc. This new family enrollment replaces duplicative efforts for families with children/students across the spectrum from Pre-K through High School. 

    Though this is “just an enrollment form”, our desire is to simplify your parent work effort and demonstrate our renewed efforts to minister to and with the whole family.  

    All communications are sent through Flocknote, therefore an up-to-date family email address and phone number are necessary for effective communication of class details and information.

    This enrollment form is for the whole family, for students of all grades Pre-K through 12th Grade, for the academic year and it includes preparations for the Sacraments. Additional paperwork may be required for sacrament preparation classes.

    *Sac Prep Year 1: Preparations for the Sacrament of Reconciliation begin for all baptized 2nd grade students or older who haven't already received the Sacrament of Reconciliation.

    *Sac Prep Year 2: Preparations for the Sacrament of Confirmation and Holy Eucharist, begin for 3rd grade student or older, who have already received Reconciliation but not yet received Confirmation and Eucharist.

    Please complete for each child that will be participating in faith formation classes either within the Family Faith Formation Sunday classes, Middle School or High School Youth groups, based off of their grade level and sacraments completed. Please indicate which sacraments each child has received.

    Saint Joan of Arc, as part of the Archdiocese of Denver, follows the Restored Order for Sacramental Preparations.  This is a 2-year preparation series where a child will begin preparations in 2nd grade and be completely initiated into the Catholic faith by the 3rd grade. 

    Year 1: Reconciliation in 2nd Grade

    Year 2: Confirmation and First Eucharist in 3rd Grade

    In addition to regular Sunday attendance at Faith Formation classes, there will be 3 required additional Sacramental Prep meetings during each term, normally held on announced Wednesday evenings.

    Minimally, one parent or guardian must attend these meetings.  We will assist you in preparing your child or children for the Sacraments, including assignments and practice.  

    Also, we will have a retreat day for the children before the Sacraments are received. More information to come!

  • Family Commitment to the Faith Formation Program

    Below are policies that families agree to abide by as part of partaking in this program.
  • Parent/Guardian Responsibilities: In addition to being the primary educators in the faith, parents by their enrollment into the Faith Formation agree to support the process in the following ways:  

     

    Domestic Church: It is important to recognize that the formation leaders and teachers assist the domestic church (your household) in training the children in the faith; they cannot replace you, nor is this faith formation program sufficient to fully pass on the faith to the next generation.  Please walk with your child every step of the way and be sure to share the beauty of the faith with them on a daily basis.

     

    Attendance: Attendance is important for all families as it provides consistency and prioritizes the faith over worldly things.  For children in Sacramental Preparation, absences may result in a delay of receiving sacraments.  In addition, all meetings and retreats are mandatory. Please contact a department leader when your child misses a class for the make-up.   

     

    Sunday Faith Formation Parent Attendance: A parent or guardian is strongly encouraged to attend the parent session every time the child is in a children’s session. Building community is an important aspect of the church, it is best if the same person or persons can be present for the sessions.

    It is with joy that the church offers the above programs for the growth in faith of our youth and families.  After you submit your registration a link will pop up. A suggested donation of $25.00/child will assist us in offsetting program expenses.  Thank you, in advance, for your generosity.
