I acknowledge that I have requested New Mobility Services Inc (DBA NEMO) to undertake a safety check on my bicycle. I understand the Longwood Collective, Inc. (“LC”) has engaged NEMO to undertake safety checks and make adjustments to bicycles owned by employees of their member institutions seeking this assistance. The service is being provided to me at no cost. This program is a part of the Longwood Collective’s “Longwood Collective TMA” program goal to encourage alternative methods of commuting to the Longwood Medical and Academic Area.
In considerations of these services, I hereby release and hold NEMO, Longwood Collective, the host institution, and all of their respective subsidiaries, owners, members, directors, officers, representatives, agents and employees from all claims, losses, liabilities, damages, injuries (including the reasonable cost of defense) arising out of, or in connection with, my participation in this program, and the work undertaken by the bike vendor. By submitting this form, I confirm I have read and agree to the statements above.

Bike tune ups will be held at 375 Longwood Ave (Plaza)
Please note: for inclement days, tune ups will take place in the office space in the 375 Longwood garage ( next to the childcare center). Please call 617 632 2796 if you have trouble finding the location.
