  • Please use this Scoresheet and the instructions below to complete your evaluation. For additional guidance, refer to the Tip Sheet which provides helpful information.

    Note: Once your submission is completed, you will not be able to make changes. Please review your responses carefully before submitting.

    Instructions

    Step 1. Read the front page of the Trustee Application Form for guidance on Trustee Requirements (e.g., minimum time in Al‑Anon), responsibilities and selection process.

    Step 2. Evaluate each applicant using Steps 3 through 5. 

    Step 3. If an applicant fails to meet the minimum “time in Al‑Anon” requirement or for any reason you feel that an applicant is unsuitable for the position for which they have applied, please select “Disapprove” and provide a brief supporting statement in the “Comments” textbox. No further evaluation is needed.

    Step 4. Use the scale of 1 to 5 provided for each section to evaluate the responses given by each applicant.

    Step 5. In the “Comments” section, provide any rationale, support, or clarification of the scores assigned to each applicant.

    Step 6. If scoring more than one applicant, rank the applicants based on their “Overall Scores” and “Comments.” A response in the "Comments" textbox is only required if you choose to change the ordered rank of applicants to be different from the calculated greatest to least "Overall Scores."
