This innovative approach allows us to provide faster, more accurate quotes without the need for medical applications, streamlining the entire process.
What We Need From You:
Step 1: Download the Census Template
Step 2: Complete the member census excel document containing the following information:
- First Name
- Last Name
- Gender
- Date of Birth
- Zip Code
- Relationship (subscriber, spouse, child, etc.)
- Coverage Type (employee only, employee + spouse, employee + child(ren), or Family)
- Current medical benefit plan (if multiple plans are offered)
