Includes: spay/neuter, left ear tip, Distemper & Rabies vaccines, and Selamectin (preventative for fleas, ear mites, roundworms, and hookworms). PLEASE NOTE: You must adjust the quantity to the number of cats entering the clinic.

PLEASE NOTE: In order to reserve a clinic date, the below criteria must be met and acknowledged by checking each box:
REFUNDS & DATE CHANGES: Refunds can only be issued if an appointment is canceled a minimum of 5 days before the scheduled date. If a date change is needed, it must be discussed with shelter staff a minimum of 5 days before the scheduled date. All no call/no show customers and cancellations within 5 days of the scheduled date forfeit their payment.

- PLEASE NOTE: The following contains important information regarding our Outdoor Cat Clinic procedures and participation requirements. Please read each statement carefully and acknowledge by checking the box.
- Is this cat(s) showing any evidence of being sick? PLEASE NOTE: All cats must be healthy in order to attend the clinic (no nasal/ocular discharge, coughing, sneezing, etc.) If they are found to be sick on clinic day, we will ask you to take the cat back with you. If this occurs, please call WCHS to have the cat transferred to the next available clinic.
- Please explain any observed signs of sickness.
- If you elected to have a FeLV/FIV test performed and the results are positive, we recommend that the cat either becomes an indoor-only cat or is humanely euthanized. If the cat(s) results are positive and we are unable to reach you right after results appear, would you like us to humanely euthanize the cat(s) while still under sedation? An additional $30 will be applied to cover the cost of euthanasia and cremation.
- I am the caretaker of the above-described cat and I authorize Washington County Humane Society (WCHS) to surgically sterilize (spay or neuter) this cat(s) and provide other related medical care ("Procedure"). I understand that this Procedure will prevent reproducing and that this Procedure is irreversible.
- I understand that this Procedure has some potential risks, including the possibility of infection, hemorrhage, adverse drug reactions and even death. I understand that although this cat(s) will receive a pre-surgical exam, this exam is not comprehensive and will not include blood tests. I understand that there is a possibility that this cat(s) could have underlying medical problems that will not be discovered by this limited pre-surgical exam and that this possibility increases the risks associated with the Procedure.
- I agree to hold harmless WCHS, its directors, officers, staff members, volunteers, and veterinarians for any problems experienced by this cat(s) because of the Procedure or the above risk factors. I agree to be financially responsible for any post-operative medical treatment relating to this Procedure or any other unrelated medical problems of this cat(s).
- I agree to be available by phone today. If a situation arises and I cannot be reached by phone, I authorize the veterinarian to use their clinical judgement as to how to proceed. I agree to pay reasonable additional charges, if any, which may arise from such additional procedures.
