This application is submitted to MedPro Supplies for the purpose of obtaining credit. The undersigned represents and warrants that all information contained herein is current, correct and complete and that MedPro Supplies may rely on such information in deciding whether or not to grant credit. The undersigned agrees to immediately notify MedPro Supplies, at writing, of any change in the foregoing information including, without limitations, any change in the nature of the undersigned's business, ownership, name or location business or financial condition. MedPro Supplies may limit or discontinue any credit at its sole discretion at any time. The undersigned agrees to authorize MedPro Supplies and any credit agency or any investigative service engaged by MedPro Supplies to verify or otherwise investigate any information contained herein, or listed references, statements, reports or other information obtained with respect to the undersigned from any other source MedPro Supplies deems appropriate.
