    When completed, press the green "Submit" button at the bottom. You should then see a Thank You message and your submission will be forwarded to the UUA for review and execution. You will receive a fully executed copy after UUA reviews and signs your agreement.

  • The Unitarian Universalist Association (“UUA”) maintains the Unitarian Universalist Organizations Retirement Plan (the “Plan”) to enable employees of congregations, regions, the national association, other UUA organizations and Unitarian Universalist (“UU”) ordained or UUA fellowshipped community ministers, as defined in the Plan, to accumulate funds for their retirement. The Plan is a multiple employer, church retirement plan that is intended to be tax-qualified under section 401(a) of the Internal Revenue Code. (“Code”). The UUA Retirement Plan Committee serves as the “Plan Administrator” and, along with its delegates, has the authority to execute this Employer Participation Agreement and implement its provisions as provided herein.

  • If you answered YES to all requirements in either Option A or Option B, you may adopt the Plan as a Community Minister. Continue to read and initial all attestations and complete Schedule A.

  • Community Minister Certifications and Attestations: (Please review and initial for each attestation where indicated.)

  • 1. Certification of Employee Census Data: I attest that along with this agreement I have submitted a Schedule A listing all current employment as a Community Minister and in the past twelve months as of the date of completion. I agree to submit a new Schedule A each new Plan Year when requested by the Plan Administrator.

  • 2. Certification of Definition of Compensation: I have reviewed the Plan definition of Compensation (Section 2.13) and understand how to compute any contributions made to the Plan on my behalf. I understand I must follow IRS guidelines for annual contribution limits.

Certification of Designated Plan Remitting Admin: I attest that I will always have at least one person (which may be myself) designated as a Remitting Admin and will maintain a secure login to the recordkeeper platform to perform all required administrative tasks, including the timely remittance of all contributions via ACH bank setup. I understand that Remitting Admin login credentials may never be shared.

  • 5. Certification of Contribution Method: I understand that as a Community Minister, I am responsible for making my own contributions to the Plan and that contribution methods and tax implications will vary based on my employment status and business structure.

    Please complete for all current employment as a Community Minister and for the past 12 months (required for all new agreements).

  • Participation Agreement Final Approval:

  • I may elect to discontinue contributions to the Plan by submitting a request to discontinue to the Plan Administrator. For the discontinuance to be effective, the Plan Administrator must acknowledge the request and provide an approved discontinuance date.

    The UUA may amend the Plan (including the Participation Agreement) at any time. The Retirement Plan Committee may also make certain amendments to the Plan (including the Participation Agreement). The UUA will notify Community Ministers in advance of any amendment to the Plan or the Participation Agreement that changes the duties or liabilities of the Community Minister. If I object to any changes to the Plan or the Participation Agreement, I may discontinue contributions to the Plan by providing written notice to the Plan Administrator.
     
    Pursuant to the terms of this Participation Agreement, I hereby adopt/discontinue the 2025 Restatement of the Unitarian Universalist Organizations Retirement Plan and all amendments, as a Community Minister under the Plan, and agree to be bound by the terms of the Plan subject to federal law and regulations.
