  • Before You Begin

    To submit the form, you will need supporting documents for all your bank accounts (chequing, saving, TFSA, RRSP, etc.) and debts (mortgage, credit cards, etc.) on the following three dates:

    • Today
    • Your Date of Separation (DOS)
    • Your Date of Marriage (DOM)

    The documents can be uploaded directly to this form. It may take you some time to gather them, however, they will be necessary in order to complete the financial information form. You can save your progress at the bottom of each page and return to it later.

    Once submitted, a copy of your responses will be sent to your email. If you need to make changes, simply click the "Edit Response" link in that email.


    Purpose of This Form

    This form is designed to help us determine the total value of your assets on three key dates. You will be asked to provide details about your assets and debts as of:

    • Today (Part 2)
    • Your Date of Separation (Part 3)
    • Your Date of Marriage (Part 4)

    The form begins with your most current financial information, as it is typically the easiest to access. From there, it works backward toward your date of marriage. You may need to contact your bank(s) to obtain the required documentation.

Exclude any accounts that have been closed, items sold, or other assets you no longer hold, as well as assets you expect to hold in the future. It is important that you only write down the values of the assets you hold as of today.

      You will need to obtain a "Family Law Value" (FLV) of your pension. Please choose your pension's regulatory body from the options below to learn how to retrieve this information.
