After Schooler Enrollment Form

Hello Parents,

Thank you for your interest in our After Schooler program! We have a new location this year for the program, which allows us to service up to 20 students.

Our After Schooler program is now located at the following address:

Kool Kidz Playground, 2010 N Hampton Road, Suite 700, Desoto, TX 75115 www.koolkidzplayground.com

Our after schooler students will receive light homework help, afternoon snack and dinner, access to board and card games, technology time (tablets only loaded with approved educational, kid-friendly apps or your child can use their own device), as well as time to play in the playground structures, which include large slides, trampoline, swings and more!!

The weekly fees are as follows:

-- $75/week for Cedar Hill Collegiate Prep Academy students (only provide after care from school)

-- $85/week for all DeSoto ISD students (we currently provide transportation services to/from KJTMA, Amber Terrace and Woodridge only)

-- $90/week for Uplift school students (due to early Wednesday closure schedule)
