  • In applying for membership, I pledge that the information provided herein is accurate and truthful. I will conform to and be bound by the bylaws of the Club as they may be amended from time to time by the Board of Directors.

    I further pledge to allow use of my membership only to permanent residents of my household and understand that granting use of my membership to a non-member (other than my accompanied guests) is grounds for immediate termination of my membership privileges.

    I authorize the Club to publish my name, position, and place of employment as well as those of my immediate family members, in its newsletter, and to include information from this application in a private, members-only website membership directory. I also authorize the Club to use photographs of me and my family on occasion in the Club's printed materials and on the Club's web pages, and to send text messages to the cell numbers listed.

    If I have selected a Golf membership. I commit to membership for a minimum of 12 months. As a Golf member, I am ineligible, for a leave of absence. I understand that dues for these upgrade options are subject to change. I further understand that there are select days when the Club and/or Forest Akers are unavailable due to tournaments, repairs, or other circumstances.

    By signing below, I acknowledge that all charges for services that are required to be paid at time of service at reciprocal and partner club facilities must be paid by major credit card or other form of payment acceptable to the participating club. I understand if the credit card company rejects any charges, I may be subject to reasonable collection fees and late charges as allowed by law. I agree to conform to and be bound by the rules and regulations of all reciprocal and partner clubs as they may be amended from time to time.

    I understand my Social initiation fee has been waived, and have enclosed the appropriate initiation fee(s), for any add-on memberships for which I have applied, and further understand that initiation fees are non-refundable and not deductible as a charitable contribution. (In the event that this membership application is not accepted, all amounts received from the applicant with this application will be refunded.)

    I understand there is a monthly food and beverage minimum purchase requirement. I recognize that dues are subject to change by the Board of Directors, and my Social dues will begin to be billed automatically at the end of the 12-month complimentary period. I will ensure timely payment of all dues and authorized charges posted to my member account, and if choose to resign, provide a 30-day notice.
