• Rocky Mountain Equality Youth Field Trip to Light Root Farm

     

  • STOP! PARENTS/GUARDIANS/ INDIVIDUALS 18+ SHOULD BE FILLING OUT THIS FORM SO THEY MAY HAVE ALL OF THE IMPORTANT DETAILS FOR THIS TRIP. 

    IF YOU ARE A MINOR FILLING OUT THIS FORM, YOU NEED TO WAIT UNTIL YOUR ADULT IS AVAILABLE TO GO THROUGH IT WITH YOU.

     

    This program is an exclusive offering to our High School Youth Group to participate in various farm-centered activities, meet baby farm animals and learn about animal husbandry, and enjoy the great outdoors. Oftentimes, groups leave the farm with fresh produce, herbs, and a better understanding of organic farming practices.

    Please make sure your youth brings the following:

    Closed toe shoes (No lightweight shoes, heels, platforms, or “five-finger” types of shoes.) - there are rubber boots available if not appropriate footwear is worn

    --A water bottle (We will have extra water.)

    --Clothing appropriate for weather conditions (Dress in layers.) 

    --Backpack or something similar, to carry personal items

    -sunblock and bug spray

    There is a liability waiver from Light Root Farm for participation in this series.  It will be sent to you via email once we receive your RSVP.

    Light Root Farm is located at: 7715 Arapahoe Rd, Boulder, CO 80301

    We will be meeting at the farm! (Subject to change based on our new 15 passenger van availability! If vans become available for transportation, transportation forms will be updated and new arrival times to utilize the bus will be added)

     

    Rocky Mountain Equality programs require that parents/guardian sign and return this form to staff before your is allowed to participate in the field trip/activity.     I hereby release, and agree to indemnify and hold harmless the owner and operator of the vehicle, Boulder Pride dba Rocky Mountain Equality, and any of their respective employees, agents, officers, representative, successors, assign, and insurers from liability for any injury, loss, or damage to my child, or property, whether anticipated or unanticipated, resulting from participation in any activities contemplated by this Agreement.

    I realize that going on this trip may involve risks and hazards, including the risk of injury, to my child. I am aware of the risks and hazards inherent in my child's participation and do hereby assume sole responsibility for all such risks and waive all claims against Boulder Pride dba Rocky Mountain Equality and their respective agents, representatives, officers, employees, successors, assigns, board and insurers.
