  • STOP! PARENTS/GUARDIANS/ INDIVIDUALS 18+ SHOULD BE FILLING OUT THIS FORM SO THEY MAY HAVE ALL OF THE IMPORTANT DETAILS FOR THIS TRIP. 

    IF YOU ARE A MINOR FILLING OUT THIS FORM, YOU NEED TO WAIT UNTIL YOUR ADULT IS AVAILABLE TO GO THROUGH IT WITH YOU.

     

    Join us at Groundworks Art Lab for another round of exploration of artistic endeavors! Crafts ranging from clay, to metal, to wood, to printmaking, and beyond await our youth!

    Dates: 

     October 15 5-7p High School (14-18 y/os)

      October 20 5-7p Middle School (12-14 y/os)

    Location: Groundworks Art Lab 3750 Canfield St, Boulder, CO 80301

    MEET AT THE ART LAB! Parking is not very abundant, plan to drop your youth off and park at our RMEQ building 0.5 miles down the road. 

    Rocky Mountain Equality programs require that parents/guardian sign and return this form to staff before your youth is allowed to participate in the field trip/activity.
