In exchange for my child(ren) participating in programs and activities at the Children’s Resource & Referral Discovery Museum (formerly Santa Maria Valley Discovery Museum), I understand and accept that hands-on, play-based learning may involve physical activity and use of public exhibits, which carry some inherent risks. Although the Museum has safety measures in place, I recognize that accidents or unexpected incidents may still occur.
I understand and accept that the museum environment may include exhibits and educational materials with small objects, which could present a choking hazard. I agree to supervise my child(ren) appropriately when present and to instruct them on safe and respectful behavior during participation.
I agree to release, hold harmless, and indemnify the CRR Discovery Museum, its staff, volunteers, and affiliates from any claims, liability, or damages resulting from my child(ren)’s participation, except in cases of gross negligence.
I understand that if my child(ren) engages in behavior that poses a risk to themselves or others, I may be asked to remove them from the program or Museum immediately. If the event is a drop-off program and I am not on-site, I agree to pick up my child(ren) within 20 minutes of being contacted. I also understand that failure to pick up my child(ren) promptly at the end of a program may result in a late fee of $1.00 per minute.
