• Personal Financial Statement (PFS) Application

  • This application is designed to help you generate a complete, lender-ready Personal Financial Statement (PFS) in the format most commonly required by SBA lenders and traditional banks.

    All you need to do is complete the questions accurately and honestly- we’ve made it as easy as possible to understand. Where helpful, you’ll find brief descriptions or examples to guide you. 


    How It Works

    The form is comprised of four easy-to-follow sections:

    1. Basic Information
    2. Financials (Assets, Liabilities, Income, etc.)
    3. Schedules (Detailed breakdowns)
    4. Declarations (Yes/No questions and final statements

     Skip any questions, sections, or entire pages that don't apply to you by clicking 'Next'; at the bottom of any page.

    IMPORTANT: Please do not use dollar signs ($) anywhere in this form. Just enter the digits- for example, type 25000 instead of $25,000.00.

     

    Co-Applicants: If your application includes a Co-Applicant who is also the  spouse/significant other of the Applicant, they will complete their portion after the Applicant finishes, within this same form. If your application includes a Co-Applicant is not a spouse/significant other (just business partner), the Co-Applicant will complete an entirely seperate PFS questionaire. If you are filling this out on behalf of both, you can:

    • Complete it straight through, or
    • Bounce around the sections and come back as needed.

     

    Joint Accounts:
    If you and your spouse are both completing this form, list any joint accounts, loans, or properties only in the Applicant’s section. This also means the Co‑Applicant will only be listing individual accounts.

    *In an effort to save space, the word 'spouse' will be used to represent both 'spouse' and 'significant other' throughout this questionaire.
