Bentley’s Country Kennel Boarding Agreement & Waiver

This Agreement is entered into between Bentley’s Country Kennel (“Kennel”) and the petowner (“Owner”) whose signature appears below, for services provided for the followingpet(s) (“Pet”):

1. Veterinary Care Authorization

Owner authorizes Bentley’s Country Kennel to act on their behalf in obtaining veterinary care at Owner’s expense should the Kennel deem it necessary.

If the Pet becomes ill or injured, the Kennel, in its sole discretion, may engage the services of a veterinarian.

All reasonable attempts will be made to contact the Owner prior to treatment; however, in the event of an emergency, the Kennel may proceed with veterinary care in the best interest of the Pet.

Any medications required will be administered per veterinarian instructions.

The Owner is responsible for all veterinary charges incurred.

2. Release of Liability

Owner agrees to release Bentley’s Country Kennel, its owners, staff, agents, and
representatives from any liability or claim for injury, illness, loss, or death of Pet while in the care of the Kennel.

Owner further accepts full responsibility for any damages to the property of the Kennel, injuries to staff, other pets, or third parties caused by their Pet.

3. Fees & Payment
Owner agrees to pay for all boarding services according to the rates in effect at the time of service.

All fees are due in full at or before the time of Pet’s checkout.

If Pet is picked up after closing time or before opening hours, additional charges may apply.

4. Abandonment Clause

If the Owner fails to pick up the Pet within five (5) calendar days of the scheduled pick-update, without prior arrangements, the Pet shall be considered abandoned.

In such a case, Bentley’s Country Kennel reserves the right to place the Pet in the custody ofthe Regina Humane Society.

Owner remains responsible for all outstanding fees.

5. Owner’s Representations

Owner certifies that:

The Pet is healthy and free of contagious diseases.

All vaccinations required by Bentley’s Country Kennel are current.

The Pet has no known aggressive tendencies toward humans or other animals.
6. Medication Administration Consent

By checking this box and signing below, I authorize Bentley’s Country Kennel and its staff to administer medications, including but not limited to oral medications, eye drops, or topical treatments, as provided and instructed by me, the pet owner. I confirm that all medications have been prescribed or recommended by a licensed veterinarian and are properly labeled with clear dosage and administration instructions. I release Bentley’s Country Kennel and its staff from any liability related to the administration of these medications.
Medication Administration Consent

I give my consent for Bentley’s Country Kennel to administer medications to my pet as instructed.
7. General Terms

This Agreement shall apply to this and all future visits of the Pet to Bentley’s Country Kennel.

By signing below, Owner acknowledges having read, understood, and agreed to all terms and conditions outlined in this Agreement.
