I give permission for Dawn’s Time to care for my pet in my absence. I authorize the assigned Dawn’s Time pet sitter(s) to transport my pet to and from my veterinary clinic or request on-site treatment if deemed necessary. I understand that Dawn’s Time may make decisions regarding my pet’s care based on their condition and in consultation with veterinary professionals, with my pet’s well-being as the top priority.

I understand that I am fully responsible for all veterinary fees and charges incurred on my pet’s behalf. I agree to settle any costs directly with the veterinary provider according to their payment policies.

I also authorize my veterinary clinic to release medical information about my pet to Dawn’s Time and any attending veterinary professional as needed. Dawn’s Time will maintain open communication with me regarding any treatment, decisions, or changes to my pet’s care.
