Dawn’s Time will make every reasonable effort, including multiple attempts, to contact me using the contact information I have provided before taking any non-emergency action regarding my pet’s care. If I cannot be reached in a timely manner and immediate care is required to protect my pet’s health or well-being, I authorize Dawn’s Time to proceed with appropriate care based on my pet’s condition and in consultation with veterinary professionals, with my pet’s well-being as the top priority.
I understand that I am fully responsible for all veterinary fees and charges incurred on my pet’s behalf and agree to settle any costs directly with the veterinary provider according to their payment policies.
I also authorize my veterinary clinic to release medical information about my pet to Dawn’s Time and any attending veterinary professional as needed. Dawn’s Time will maintain open communication with me regarding any treatment, decisions, or changes to my pet’s care.
