Your learning doesn't stop at the door each participant receives four weeks of structured post-course follow-up to continue refining clinical reasoning and application. Designed to go beyond the weekend and into real practice.

    • Participant Liability Waiver & Course Agreement

    • COURSE PARTICIPATION AGREEMENT

      This Agreement (“Agreement”) is made between Rehab Code, LLC (“Provider”) and the undersigned Participant, who voluntarily agrees to the following terms as a condition of participation in the live course (“Course”).

      1. ASSUMPTION OF RISK & RELEASE OF LIABILITY
      The Participant acknowledges that the Course involves manual therapy, hands-on learning, exercises, training, strength testing, and other physical activities, all of which carry inherent risks. The Participant voluntarily assumes all risks, including but not limited to personal injury, property damage, or adverse physical effects resulting from participation.

      The Participant waives and releases Rehab Code, LLC, its instructors, employees, affiliates, and agents (“Released Parties”) from all claims, liabilities, or damages, whether due to negligence or otherwise, including but not limited to injuries sustained during exercises, training, strength testing, or any other Course-related activity.

      The Participant agrees not to hold the Released Parties responsible for injuries sustained during the Course or from applying Course techniques in any setting.

      2. PARTICIPANT RESPONSIBILITY & INDEMNIFICATION
      The Participant agrees to:

      Use professional judgment when applying Course techniques.
      Ensure partner safety during hands-on sessions.
      Avoid using techniques in a way that could harm others (patients, colleagues, etc.).
      The Participant indemnifies and holds harmless the Released Parties from any claims, injuries, or liabilities resulting from their participation or application of Course content.

      3. INTELLECTUAL PROPERTY
      All Course materials (manuals, slides, videos) remain Rehab Code, LLC’s intellectual property. Participants may not reproduce or distribute materials without written permission.

      4. SOCIAL MEDIA RELEASE
      The Participant acknowledges and agrees that Rehab Code, LLC may capture photographs, videos, or other media during the Course for educational, promotional, or marketing purposes.

      By signing this Agreement, the Participant grants Rehab Code, LLC and its representatives the irrevocable right to use their likeness, image, voice, or statements in any media format, including but not limited to social media, websites, promotional materials, and course content.

      The Participant waives any right to compensation, royalties, or approval of any such use. If the Participant wishes to opt out of media use, they must notify Rehab Code, LLC in writing prior to the Course.

      5. GOVERNING LAW & DISPUTE RESOLUTION
      This Agreement is governed by the laws of Arizona. Disputes shall first be resolved through mediation in Arizona. If mediation fails, disputes shall be settled in an Arizona court.
